Shift rail wear and poppet balls/springs, wear allowing end play on 2nd gear and/or wear on the cluster gear with excess end play. Once 2nd starts moving...it is hard for that poppet ball and spring to keep it in place.The cases are repairable and they are dated too...so you probably do want to keep the original. It can be fixed!

John Hedges Thanks for the comment John.100% on the brass drift. I forget I have them!As for thread chasers - I have never owned a set nor have I ever been in a shop that had a proper set. Your comment sent me down a rabbit hole....turns out they are not very common. I see a couple of different options out there so need to do more research. I totally agree that not removing material while repairing threads is the ideal outcome....will report on my findings in an upcoming episode.Good luck getting that transmission back together.
